Patient Forms

Upon entering our office, one of our staff members will  welcome you as a guest.  We will request that you complete a few patient forms so our Chiropractor can see whats going on with all your body systems.

Health History

Next, you will have a consultation with Dr. La Starza to discuss your health-related concerns. This initial visit is designed for Dr. La Starza to learn more about you, your condition, and  expectations to determine how chiropractic care can meet your health goals.

Examination

After your consultation, Dr. La Starza will perform a complete chiropractic evaluation.  The evaluation includes a Stress body scan, motion palpation, ranges of motion, postural analysis, and muscle testing to see how you are functioning.

X-Ray Studies

Depending on your specific concern, and what the doctor finds, we may take x-rays. X-rays help us develop  the most effective plan of care for you, and to see if Chiropractic can help you. You may bring in your own x-rays, and Dr. La Starza will look to see if any other imaging is  necessary.

Report of Findings

Once the information is collected  and examinations are performed, Dr. La Starza will give you a detailed report of all findings and answer any questions including:

1. Can Chiropractic Care help me?
2. How long is it going to take?
3. What are my options?
4. What is the cost of my care?

Dr. La Starza will see where you are at with your current state of health and where you need to be.

Chiropractic Adjustments

 At the conclusion of the report of findings, you can choose to experience your first adjustment.
